When we were "passed off" we didn't always feel like we knew the plan. Things could have been communicated better. There was definitely lack of planning during the Hoi An Express bus transport. As for extra days, you can spend 3 days in Sapa or Pu Luong - Ninh Binh: Option 1: Ha Noi - Lao Cai overnight on train, Sapa - Muong Hoa - Lao Chai Tan Van, Sapa - Fansipan - Cat Cat Village, Sapa - Free time - Ha Noi by limousine. Option 2: Ha Noi - Pu Luong - Bamboo Rafting on Cham River, Pu Luong - Kho Muong Village - Lan Village, Pu Luong - Ninh Binh - Hoa Lu, Ninh Binh - Trang An - Mua Cave - Ha Noi.
